Keeping The Records Policy and Procedure

Staff must report all communication with clients and/or authorities to the supervisor/manager by recording it in the logbook located in each room. 

Any incidents and/or observations involving children needing to be communicated to parents and management, must be recorded in the logbook immediately. 

These would include:

  • ANY concerns about children`s health, 
  • sleeping pattern changes, 
  • toileting and diapering issues, 
  • developmental concerns,
  • ANY rash on a body, 
  • ANY suspicions in regards to diet and nutrition, water intake, 
  • ANY bruises, scratches, pinched skin, falls, biting, etc. 
  • Staff changes-change of room must be noted in log book of room left

Report to the manager:

  • absences, 
  • illnesses, 
  • changes in shifts, 
  • any complications in daily work as a team, 
  • reporting supplies we are getting low on (art, cleaning, etc), 
  • broken or damaged toys/books
  • any other required repairs in a room.   

Incident Report Procedure

In case of child being bit, scratched, pinched, bumps, falls, etc. The teacher who was directly dealing with the situation must fill out a Minor Incident Report form and mark it in the logbook “see incident report”.

Staff must record any incident in the logbook and fill a Minor Incident Report form to the best of his/her knowledge.

Staff (not necessarily the one who did the report) must submit the Minor Incident Report form to the parents at pick up time to be reviewed and signed. 

Staff is required to answer the questions and follow the procedure of writing a report in the logbook when talking to the parents about the incident. 

Signed form is to be submitted to the manager. The form will be kept in child`s file in the office. 

Reporting in the Logbook Procedure:  

When writing a report follow the next steps:

  • Briefly describe what happened, when and at what time
  • Who was in the room (teachers, parents, authorities or visitors)
  • Why it happened  
  • What has to be done to prevent it from happening again 
  • How parents can help (if applicable)

Logbook is confidential and is only for the personnel of the facility and licensing to review. Make sure it is put away properly. Logbook is not to be reviewed by parents or with the parents.
